TinyMCE Icons Not Showing

The icons for the TinyMCE editor that is shown when you reply to a topic is showing little squares where all the editor icons should be. How can I fix this?

This must be caused by another WordPress plugin or theme which tries to replace the original icons with custom ones – maybe by using third-party icon-packs. Please try to disable other plugins or switching to one of the default themes for testing purposes to find out which component is causing this bug. After you have found out which component is responsible for this, try to contact the plugin/theme author for a possible fix or workaround.
