Duplicated breadcrumbs

The duplicate breadcrumbs are caused by another plugin or your theme. Please try to disable other plugins for testing purposes and switch back to one of the default themes to find the component which is responsible for this behavior.

The URL does not contain the word forum twice. The first forum term is the name of your WordPress page. You can change its slug via the WordPress backend. The second forum term is the view – so when you are inside the forum, forum is shown so the system knows where you are.

Issues like this can be caused by a lot of plugins. Its hard/impossible to say which kind of plugins can cause this issue because there are millions of plugins and themes available for WordPress. The easiest way is to just disable one plugin, test if the problem is gone, if not activate it again and continue with the next one. I suggest you to start with plugins which are doing some complex under-the-hood-changes like some user-access-management extensions, etc.
